MONDAY, MARCH 13, 2017 ® REGISTRATION BEGINS
We are looking forward to having your child attend our school. Registration for Kindergarten and Transitional Kindergarten will begin on Monday, MARCH 13,2017 through the remainder of the school year. Your child must be five (5) years old on or before September 1, 2017. If your child turns five (5) years old between September 2, 2017 and December 2, 2017, they qualify for Transitional Kindergarten. This bulletin explains the requirements for registration.
NOTE: Your application will be accepted ONLY if you bring ALL the items listed below and all forms are filled out completely and signed where required.
- Original certified live birth certificate, baptismal certificate or passporte
- Proof of address with parents name (Utility bill, mortgage, federal or state mail, proof of residency)
- Immunization record consisting of:
MMR - 2 doses, both given on or after the first birthday
POLIO – 4 doses at any age (3 doses are accepted if 1 dose was given after 4th birthday)
DTP – 5 doses (4 doses are accepted if one was given after 4th birthday)
HEPATITIS B – 3 doses
CHICKEN POX – 1 dose if child has had disease (Physician or clinic should mark “had disease” and stamp form)
- Parent/Adult – TB Parent or adult is recommended to participate in classroom and must provide an updated tuberculin test.
- Completed Nutrition form
Dental – Dental checkup is required by May 18, 2018. You may pick up a form in our office.
Physical Examination – State law requires a physical exam during the kindergarten year before entering 1st grade. Please pick up an orange card from the office and take it to your child’s doctor to be completed and returned to our office.
Class requirements – All students are required to come to school every day and on time and in school uniform. A parent or adult must accompany the student daily to support the student with the “Young Scholar’s Workshop“program.
